Return completed document to RJMW with requested documents and license copies and a current copy of your resume.

Current detailed resume
Copies of all current licenses
Copies of any/all certifications secured
Completion of the RJMW Independent Contractor’s Agreement
Digital photo (head & shoulder shot)
Copy of passport (if applicable
